Waiver Support Coordinator

Coordinating Solutions Inc is seeking motivated, compassionate professionals to join our growing team as Waiver Support Coordinators (WSCs) and Consumer Directed Care Plus (CDC+) Consultants under the Florida Agency for Persons with Disabilities (APD).
About the Role
Waiver Support Coordinators are licensed APD providers who serve as case managers for individuals with developmental disabilities. In this role, you will help individuals and families navigate complex systems, access critical services, and build support plans that reflect each person’s unique goals, strengths, and needs.
WSCs play a vital advocacy role: coordinating care, ensuring compliance, managing budgets, and connecting individuals to both state-funded and community-based resources that enhance quality of life.
What You Will Do
Client Advocacy & Support Planning

Advocate for individuals and families to ensure equitable access to services and supports
-
Develop, implement, and monitor individualized, person-centered support plans
-
Collaborate with individuals, families, providers, and community partners


Service Coordination & Monitoring
-
Coordinate services across multiple providers to ensure quality and timeliness
-
Conduct required visits, assessments, and reviews
-
Monitor outcomes and adjust support plans as goals or needs change
-
Maintain accurate, timely documentation in state-required systems

Resource Development
-
Connect individuals to paid and unpaid providers, community programs, and social services

Utilize tools such as the APD Resource Directory, Florida Navigator, and local networks
-
Build sustainable, creative solutions tailored to each individual’s goals

Budget & Compliance Management
-
Manage annual client cost plans in alignment with program guidelines
-
Ensure compliance with state and federal requirements, including CDC+ regulations
-
Support individuals in understanding how resources and funding are used

Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
Note: Master’s degree in a related field may substitute for one (1) year of experience
Minimum of two (2) years of paid, supervised experience in developmental disabilities, special education, mental health, social work, or related rehabilitative services
Must have a valid Florida driver's license, reliable transportation, and the ability to travel throughout your assigned region
Must be able to pass a Level Two Background Screening Clearance for the State of Florida
Preferred Qualifications

Familiarity with the iBudget Waiver and Medicaid-funded services

Ability to remain calm and effective in high-stress or crisis situations

Strong understanding of the needs of individuals with developmental disabilities

Excellent communication, problem-solving, and organizational skills
-
Ability to manage multiple cases while meeting documentation requirements

Cultural sensitivity, professionalism, and ethical decision-making
-
Spanish fluency
